Gentamicin
Time to steady state: 1 day (longer in renal impairment).
Sampling time and target range
6-14 hours after dose: see plot in Gentamicin dosing guidelines.
Comment

Dose depends on renal function (refer to Prescribing Step 1 in Gentamicin dosing guidelines).

It may be appropriate to check peak and trough levels in some patients (e.g. patients with high body weight). Aim for a peak (1 hour post-dose) >10mg/L and trough <1mg/L.

For synergistic dosing and monitoring of gentamicin in endocarditis, see the 'Synergistic Gentamicin for Endocarditis in Adults' in theĀ NHSGGC Clinical Directory.
