Thromboprophylaxis - low molecular weight heparin (LMWH) dose guide in renal impairment

Table 1: Prophylactic LMWH dosing

**If CrCl <30ml/minute and prophylactic LMWH treatment has continued for ≥10days, reassess anti-Xa activity after the 10th dose of LMWH, to ensure there has been no significant drug accumulation (target 4 hour peak level: 0.1-0.4units/ml).
